*Terms and conditions apply - By entering the survey and completing your details, you are authorising us to ensure you can obtain your free eye check, which is available through OPSM Cannington and available to Medicare card holding customers only, and to ensure you obtain a free car safety, subject to availability through Auto Masters Cannington. You agree and acknowledge that we may have to securely share your basic details (name, phone number or email address) to ensure that they can organise to book you in to obtain your free booking. However, you understand that you can complete the survey without this being provided and can opt out at any and all times. This is not a pre-requisite to win the holiday prize as part of the survey completion. If you enter your details only to enter the competition they will not be shared with any 3rd party and will be stored securely for the duration that they are required. Booking online for free services is available however further terms and conditions apply at OPSM and Auto Masters that must be accepted by customers to procced to receive extra services and benefits from the supplier outside of the initial free booking supplied by APIWA. APIWA takes no responsibility for losses, costs or any other fee or charge associated with these services after you have contact or made an appointment or booking with the supplier and our relationship ends at the date of contact between the customer seeking to receive the free offer component and the supplier taking the booking. The competition starts 13.30pm AEST 1st December, 2022 and ends 13.30pm AEST 1st February, 2022. A winner will be chosen at random in the presence of independent witnesses. The prize is for 4 days, 3 nights for a family of 4 at the API owned property location in Kalbarri, called APIWA Pelican Beach Front Apartment. The prize is not redeemable for cash and it includes accommodation cost only, not travel or food & beverage, no other extras are included. Prizes are valid for 6 months, cannot be cancelled once booked and be secured and completed prior to September 1st, 2023. The prize is non-transferable, non-refundable and not exchangeable for cash.
